Service Agreements between the Disability Services Commission and funded organisations outline contractual information related to amount of funding, length of the funding term, type of services funded and number of people with disability that will benefit from the services funded.

In this section, service providers will find information about contractual reporting requirements to assists in the planning, reporting, and reconciliation of outputs and outcomes to ensure effective services are purchased within an environment of finite resources. 

  • Service Agreements explains what a Service Agreement is and links to the State Supply Commission’s Procurement Policy and a sample Service Agreement. The sample service agreement contains up to seven schedules on each service type
  • Serious Incident Reporting includes the Notification of Serious Incident Guidelines and the Serious Incident Report form.
  • The Contract information officers section explains the role of the Service Contract and Development Officer (SCDO).
