Do you want to refer a person to the NDIS?

WA National Disability Insurance Scheme (NDIS) referrals

If you wish to refer someone who could be eligible for the WA NDIS, you can submit a written referral on their behalf. Referrals should only be submitted with the written consent of the person or their guardian.

Carers, general practitioners, health professionals, and service providers, for example, can refer a person with disability (including psychosocial disability) to the WA NDIS by completing the WA NDIS Referral Form below and submitting it to the person’s nearest Local Coordination office. Please read the WA NDIS eligibility requirements carefully prior to submitting a referral form.

Alternatively, you can submit the WA NDIS Referral Form to WANDIS@dsc.wa.gov.au or contact 1800 996 214 for further information.

Note:

  • The person being referred (or their guardian) must give written consent for the referral.
  • The WA NDIS will be available to people at different times depending on which local government area they live in.

Local Coordination referrals

If the person lives in an area where the NDIS has not yet rolled out, they may be eligible for Disability Services Local Coordination support in the meantime.

If you would like to refer someone who lives in a non-NDIS area to assess their eligibility for Local Coordination support, please complete the Local Coordination Referral Form below and submit to Admin_LC@dsc.wa.gov.au.
