The Combined Application Process (CAP)

The Combined Application Process (CAP) is the method used by the Disability Services Commission to offer individualised funding. It provides the opportunity for a blended solution, and provides a single application point.

It is essential that you contact your Local Area Coordinator or a funded service provider for assistance in completing the application form.  Contact details can be found in Your local support and the Directory of service providers.
